I need to develop a simple mis type web application.. where in the front end the users will enter information through a form. Data will be saved in the database and will be managed through the mis. The client has just need to give the requirement, a domain name and a hosting space. What amount should i asked for doing the work ?

Quickest way: 1) Figure out how many hours it will take you to complete. 2) Determine what the value of 1 hour of your time is. 3) Multiply those 2 numbers together.
That's what you should ask to do the project. However, you also have to factor in overhead, the rates of your competition, and how desperate you are for the project.

Unfortunately i don't have any idea of the value per hour. Till now i have not taken any project as a freelancer. I have worked for 5-6 sites but that is based on instructions with a fixed monthly salary.

Well, until you determine how much you are worth, it's going to be difficult or impossible to quote a price.
You can start by looking at your fixed monthly salary and dividing by the number of hours you worked- that will give you a rough estimate. Then factor in taxes, overhead, etc.
Till now i have not taken any project as a freelancer.
Because of this, you will most likely need to lower the above rate quite a bit until you get the experience to show you are worth more.
