Hi folks

Right not sure if this is the right place but ..

I am looking for a survey that i can add to the clubs web site capable of maybe 10 questions with 3 choices for each answer i have once again found one lurking on the hosting site but it only handles a single question with 3 choices for the answer written in perl and whilst i can get more that 3 questions i am completely unable to get more that one set one answer choices .

Anyone ever found any thing or got ideas please
