Need help creating a form re-direct?

Hello,

I have a employee in my office that is working on an internal website for our customers and have come across an issue.
I am sure it is simple but I have been out of the website creation game for way to long now and can not give any ideas to help out, nor even what to search for online.

What we are wanting to do is the following: Have a link setup on our page that is once clicked will open a dialog box that will re-direct the user to a unique URL based on what is typed in the form.

Would like the action tree to be as follows if it is easy to do:
1) User clicks on button on page that opens up a dialog box asking for their domain name/last portion of e-mail address.
e.g.:
webmasterworld.com(.org/.net/etc)


2) Once filled out and submitted it will generate a url based on what was typed, then either re-direct them and or open up a new window with their unique url.
e.g.:
http://mail.domainname.com(.org/net/etc):32000/


Hope someone understands what I am going for here, and any help would be appreciated.

I'm not quite sure I understand the question. Can the form contain the url or email address and does the generated url need to exist?

So if a user types in @example.com would the link created be www.example.com?
