Form creation for end user

I'm working on a site where the client wants to be able to tick a series of tick boxes and at the end of that a form is generated for publication on the website, based on what has been ticked. The form then goes live and sends the information back to the client.

Are there any existing apps that can do that and that I can host on my own web space? Has anyone come across apps like this before?

I've dont things similar to this before - but do not recommened activating the forms in real-time... there could be all sorts of security issues with this approach. It is hard to say to much with so little information though. Perhaps I have misunderstood your request though.
