Virus on our sites homepage: how did it get there?

The company that I work for was recently informed by somebody that tried to view the source of our homepage that it prompted him about a virus.
We looked into it and found a 1px by 1px Iframe from <another site>.
It's since been removed, but I was wondering if anybody had any idea how it could possibly have gotten there or had a similar experience.
We design and host websites, so we want to make sure that this doesn't happen to any of our clients.

Sounds like your website was Hacked. Your host and / or content management system should have recommendation or be able to indentify known vulnerabilities.

This is a question for any others more than an answer.

The ftp login and password you use, are purely for the server you login in to. Is the ftp protocol secure on the internet or is it like many emails not secure and you need an encrypted or 'secure' version of ftp?
