What's the problem?
As in description I have a problem with this Input Tag Accessibility Attributesdialog box.
At the moment I'm learning about using Dreamweaver with PHP and every form, text field and so on is created (in the book I'm learning from) through this dialog box. But in my Dreamweaver I don't have this dialog box. Instead I have only a Tag editor, which as far as I know should be proceeded by this dialog box. It really complicates the learning proccess. :/ I know that I need to set this on Accessibility page in Dreamweaver Preferences but nothing happens.
I was reinstalling Dw quite a few times and only once I got this dialog box, but after two or three forms inserted it disappeared. I have the same problem both in Dreamweaver CS3 & CS4 (trial versions).
Could anyone help, please, I couldn't find an answer anywhere on the internet.

Selsky, are you using the mac version of DW?
I'm not sure if that has anything to do with anything, but I'm using Mac. I got the correct dialog box (Input Tag Accessibility Attributes) at the beginning, but now I'm always getting the 'tag editor -form' dialog box, which although it has the SOME of the same settings, it's not the same as the book's instructions so I keep wondering if I'm really doing things right. :S
So if anyone knows why this is happening it would be awesome :)

Exactly this Input Tag Accessibility Attributes dialog box should show up before the tag editor, what is why you have the option in Accesibility setting if you want this specific dialog box for Form Objects, Images etc.
I'm gonna try posting this problem on other support forums. If I get any info, solution or anything else that could be helpful, I'll post it here.
