My question is: Is there a way to insert includes to multiple pages simultaneously or do I have to insert in to each page individually?

If you are using FP2003, the Find and Replace feature is pretty powerful. You'll need to find something around the area where the includes will be placed that is constant (the same across all pages). You would then set up your find parameters from that. Then you would set up your replace parameters to include that static code and then your include code.
If you are not that familiar with the find and replace routine, then I might suggest opening each and every page and insert the appropriate code. Think of it as "spring cleaning" and while you are back there, you might as well address any other issues. ;)
You could easily do 10-15 pages per minute. It's really not as bad as you think it is. Once you get the routine going, you'll be done in no time. Be sure to double check what you are doing periodically, like every 20 pages, to be sure something didn't go wrong somewhere.
Do the copying with Windows Explorer or a file manager, not FrontPage, and the links in the copied pages won't be altered.
Note: If using the SSI method then be aware that FP won't "propagate" future changes in the include page.
