Why AMP version of my page look weird

While the normal version looks ok, the AMP doesn't look good.

I have AMP plugin installed on my project website. In AMP version, I cannot see the contact forms. It looks bad when we have a heading as "Contact us" and there is nothing beneath it. For example in one page there is a form which does not show in the amp.
Even other things too seem to appear in a single column type. What to do. please advice. How to make it look normal with showing contact forms too.

Since you mentioned using an AMP plugin, I am guessing that your site is a WordPress platform site. Since you are using a plugin, does it offer you any instruction for adding forms? Have you read the documentation for AMP and implemented iframe forms?
