Adsense Sending KeyWord Ads to Directly Navigated Page

The top line of the ad is like {KeyWord:Example Example2 Example3}
This is no good for us as publishers and no good to the advertiser.

If you literally see those brackets in the ads, it means that some advertiser tried to use so-called "dynamic keyword insertion" in AdWords, and then launched it on the GDN. Either the advertiser messed something up or there was an error delivering it.
It's a self-regulating system, so the advertiser will notice sooner or later.
