I am setting up new ad code for a new site that I am working on. In the past I have always used the code straight out of the box with all the default settings.

What are some of the setting that should be considered and why?
Display type, I will certainly use text& display, I there a reason why you wouldn't?
Is there a benefit to customizing the ad style?
Do you use custom channels how are they structured, by page, ad position, by sub-directory?
What do you use for back-up?

My ads will responsive.
I will implement page level ads.

I always run text & display. I have done a lot of testing on styles and settled on one that I use consistently; it's blended pretty well. I use channels for colors, placements, and for my event sites, locations (states), but not pages or directories (I use Analytics for measuring that). For backup at the moment I'm using a plain white html page (with analytics code) so I can measure how often the ads aren't loading, but other times I've used affiliate ads or my own banners.

Do you use channels for advertiser targeting at all?

plain white html page (with analytics code)

This sounds like a good idea, I am going to have to try it out.

Yes. No idea if it works, but it doesn't cost anything but time.
