I made no major changes on my websites for the past two years and suddenly now I'm violating Google Adsense terms & conditions and poses a significant risk to our advertisers (which I have no idea of).
The money lost is heart breaking but what's more scary to me is how Google treated their long term partners. Google had grown so big and I'm sure the big guys at Mountain View think individual webmasters like us don't matter to them anymore.
To others who are still holding an Adsense account, learn from my lesson - do not keep your money in your Adsense account for too long. You never know when Big Brother feels like hitting you for no reason.
Before anything, it's a good idea to read Google's AdSense Policies [google.com] and their Terms and Conditions [google.com].
If you are responsible for the invalid clicks, guilty as charged, then you may want to consider other means of monetizing your website. Otherwise, if you are genuinely innocent then here are some posts that may be useful to you.

There's not much we can do here to diagnose why you were banned. If you read the above posts you should get a good idea of how to proceed in getting reinstated if you feel you did not break any rules. And make sure you know the rules. ;)
