I was successfully running my adsense account for 2 years, recently I wanted to change the payee name and made a request for change in Payee name. Google asked me close existing account and apply for the new account. I did the same as per there instruction. Now they are simply not approving my site, I've applied 5 different times and all I get from them is "unacceptable content", "under construction site" etc. I'm surprised site that is up for last 1 year and was running successful campaign is now unacceptable to them. I also tried opening account with other sites of mine, still no luck.

To expand on Zett's wise words, that's certainly true if you are sailing close to the wind.
Almost all of Google's activities, especially Adsense, are almost entirely automated.
So if you have have been flirting with the extremes of the TOS (unknowingly or not), Google is unlikely to be aware unless someone reports you.
But if you choose to involve human beings in your activities, then all may be revealed. And you often cannot put the Genie back in the bottle*
Just because you have been 'getting away with it' for a while does not necessarily mean Google approved; they may simply have been unaware. Now they know.
*Opinion is divided on why Genies will not go back; it may be stubbornness, the small print of the TOS in 'Tales from the Arabian Nights'. Or it may be a function of Boyle's Law.
