I mentioned abit about this on my adCenter Blog post last week:
"Why are my ads/KWs being rejected?
To create a great user experience for Live Search we need to try and make sure that when people search for something, the ads on the results page are relevant, and when they click through to your website, they find what they’ve been looking for.
We have systems in place that check for relevance. Sometimes KWs/ads are rejected because of inappropriate content or because they do not adhere to our editorial guidelines.
If any KWs or ads are rejected you’ll be sent an e-mail letting you know and pointing you to our Editorial Guidelines. If you feel they really should have been accepted, please contact Microsoft adCenter Support and ask for an editorial appeal template. Fill it out and send it back and we’ll have one of our editors take a look. The process should take around five days to complete."
I'm concerned you see a 404 Error from links in the rejection email:
[advertising.msn.com...] [advertising.msn.com...]
Do check these links out and if necessary submit an appeal. Also please let me know if you're still seeing the 404 behaviour.

adCenterEU and I have taken the editorial issues raised here on the forum to the adCenter team to get some guidance for you on the issues some of you are experiencing. The team is constantly working to improve the editorial process because we know that it can be a pain point for customers - while also ensuring we offer a high quality user experience, and do not display any irrelevant or inappropriate content.
Our guidance to you is to contact the adCenter support team. They will be able to help you on an individual level, which is not always possible here on the forum. Because the rejection reason varies from customer to customer, and is unique to the individuals' keywords, ads, landing pages, etc., I encourage you to contact Support where they can help you with your specific issue.
That said, I'd like to reiterate that we're making sure the adCenter team is getting your feedback on this topic, and we're working to improve this process. We are piloting a solution to help alleviate some of the issues you are seeing and shorten your workload to get KWs/ads live in the event they get rejected.

Either the editors or completely inexperienced or this is being done by a software program that needs a bit of work.
I'm sure what ever the problems are they will be corrected overtime.
I remember when GoTo.com (Overture.com) first started, there were a lot of problems in the start.
Looking forward to MSN improving and welcome an alternative to Google and Yahoo.
Forcing us to manually go through every keyword and enter it on an appeal form is so time consuming - both in labor and in waiting (and based on earlier comments most likely to be an endless loop) that most rejected keywords are likely to end up never getting appealed to a human editor.
It unrealistic to force us to select each match type for every keyword individually. For those qualified for the Quick Launch program you can request this be done for you prefer not spending your time and can stand the wait.
It is even sillier to have to set the days/hours you want ads to run at the order level. Most advertisers who will want to do that are likely to want the entire account to run that way so that option should be available at the account level too.
The one client I have who wants to do that has almost 1,000 orders. Thankfully the Quick Launch team figured out how to do that too as I am certainly not going to spend my time going into every order to set the days and hours manually and the client would scream at the cost.
Having to deal with MSN AdCenter is the first time I have seriously considered outsourcing a task to India. I have dozens of clients running at Google and Yahoo who should be on MSN. I haven't pursued getting any of the smaller ones there yet because I don't want to eat all the time I would end up losing doing it.
Hopefully Microsoft is aware of the enormous amount of money they are leaving on the table every day that this system exists with such major issues. Diverting more resources to resolving them is an investment that would more than pay for itself almost immediately.
