Where is an "Unknown Region"?

I noticed this morning that I'd had 18 page views from an unknown region, checking back to January 1st I had three such visits in March but with no page views however this month, since 13th June, I have had page views from "Unknown Region" every day, so far 140 of them, 98 of them Friday 21st June alone.

No earnings have been made from any of them.

What's this likely to be, someone surfing with a satphone or something?

Probably a new IP block some ISP just put in use.

One way to not have Google know your geolocation is to use e.g. a satellite telephone (the VSAT operator will know where you are, but Google and the like won't be able to tell without the help of the satellite operator)
