The definition varies from culture to culture on what is okay for everyone to watch, the rest of the world may be somewhere in between. So for a world audience 1 scale of grading on what is mature and what is not will not work.
The undergarments of yesterday were big and covered most of the body while today's undergarments cover hardly anything!
Considering all this how to determine what content is acceptable for Google Adsense for a website that has audience from across the world ?

I am really not sure if Google themselves know what is mature content, because each area and country have different customs and traditions and have different opinion on what is mature and what is not mature !
If you use Google services, Google's definition of mature content is the only one that matters, there's no room for debate since it is their rules.
Play it safe, if you wouldn't be comfortable showing a site to your grandmother, Google wouldn't like it either.

1. Nothing
2. Ads won't show on that page
3. You get an email from Google telling you that you have questionable content. If they're really nice they'll give you a specific page to reference.
4. You'll be banned from AdSense.
I do think they have "loosened up" a little on their policy but it's still open to their interpretation. Personally I got rid of questionable content or didn't show Google ads on those pages.
I definitely agree that Google is not having as much a strict policy as earlier it used to have.
Also I have seen sites where Adult content and nudity is present and when such videos do come up, Google ads show public service ads. However, the earlier view that Google disapproves a whole site if it contains nudity in any part of the site is definitely proved wrong.
Again what is decent and indecent is relative. Undergarments seen during a tennis game or swimming or sport may be considered as nothing indecent while if anybody is seen in the same dress in a press meeting or in the parliament it is indecent.
Youtube has plenty of videos that definitely would be considered mature by many people across the world including by those in US, but then Google Ads do appear on those pages unless it has been specifically categorized as adult.
Once upon a time I think Adsense were not appearing in pages with Bikini and Sexy content whereas now I think Adsense is found in lot of sites where which has bikini and sexy contents and people in undergarments provided at least they have a bit covering the most 'vital parts.'
Again, a larger site which has 90% other stuff and 10% sexy content may get away while a site that has 90% sexy content and only 10% other stuff may be penalized, especially if someone is going to click and report the ads.
