Why does Google put the images only at the bottom? Shouldn't it give much higher CTRs?

Not sure. If your Adsense for that particular block is set to "text/image", then it's possible that images do appear there from time to time.

Not necessarily. There still is such a thing called "ad-blindness" and I believe it is more prevalent with images than with contextual ads.

That would have to be a judgement called based on your site, visitor movement, type of content, etc.
While I wouldn't place an image ad above an article, I do often place affiliate 250x250 blocks in the middle of an article (positioned to the left).
However, if you strictly specify AS image ads, you might find less ad inventory than on the text side.
